  3. Photocopy of applicant's identity (KTP) and proxy if authorized, which has been matched with the original by the counter officer;
  4. Photocopy of deed of establishment and legalization of legal entity that has been matched with the original by the counter officer, for legal entities;
  5. Original certificate;
  6. Site Plan/ Site Plan from the local Regency/City Government;
